The Mount Forest Family Health Team offers health care professionals the opportunity to work in an interprofessional primary care team dedicated to providing evidence informed care to over 12,000 patients who live and work in Mount Forest and the surrounding rural communities.

Mental Health Therapy with the Mount Forest Family Health Team is integral to the health and wellbeing of our patients. We are looking for a dedicated Mental Health Therapist who thrives in a collaborative, publicly funded health organization and believes in equitable, comprehensive community care.

What You Will Do:

As an integral part of our interdisciplinary team, you will provide comprehensive mental health services to our rostered patient population:

• Deliver short-to-medium-term, using evidence-based psychotherapy (i.e. CBT, DBT, ACT, EFT, etc.) to individuals, couples, or families.

• Collaborate directly with family physicians and nurse practitioners to create integrated, holistic care plans for complex patients.

• Facilitate mental health psychoeducational groups or workshops tailored to community needs.

• Utilize our integrated Electronic Medical Record (EMR-PS Suite) system to chart efficiently, ensuring seamless communication across the healthcare team.

Who You Are:

We are looking for clinicians who value systemic health equity and thrive in a team environment.

• Registered & In Good Standing: A Master’s degree in Social Work (MSW) or Counselling Psychology, with current registration in good standing with the OCSWSSW or CRPO or equivalent.

• A Collaborative Communicator: You enjoy stepping outside of a siloed therapy room to consult with medical colleagues for the benefit of the patient.

• Systems-Oriented: You understand the value of public healthcare, appreciate supported workflows, and view comprehensive documentation as an essential tool for patient care.

• Boundary-Focused: You are looking for a primary clinical home and are committed to maintaining clear ethical boundaries regarding public service and private interests.
