Healthy @ Home, is an innovative virtual program designed to help you learn more about a variety of health and wellness topics. You will get regular coaching from health professionals as well as the support and resources you need to see results and reach your own personal wellness goals. This healthier living support program is designed to help you prevent chronic diseases like diabetes and heart disease, and to better manage existing chronic health issues.
Through this program, you will receive:
Access to coaching support from qualified clinicians from the comfort and safety of your home.
Email and/or text “nudges” from your coaches to help you stay motivated and on track each week.
Information on nutrition, activity, exercise, chronic illness prevention and management, mindset, stress management, etc..

How does this program work?
After you are signed up, you will receive emails, text messages and phone calls to support you on a wellness journey of your choice. The messages will give you options to learn more about a wide variety of subjects like nutrition, activity, stress, anxiety, and much more. YOU are in control of what you receive and what topics you wish to learn more about. You can choose to be supported along the way by registered clinicians (via email, text and phone calls) who provide you with information and coaching to help you establish and then achieve your own personal goals.
How long does this program run for?
The initial service is a one-year program, which may be extended, to typically 3 to 5 year terms.
Why would a participant sign up for the program if it was offered to them?
If a participant wants to learn more about how to improve their health and wellness, need support with their personal goals, feel it’s time to make a change or just want helpful tips, Healthy @ Home can connect them with the resources to explore health and wellness at their own pace.
